Your First Visit at Plainsview Physiotherapy
Plainsview Physiotherapy, providing physiotherapy in Burlington, Ont., welcomes patients with a full assessment of their injury or condition followed by a customized treatment plan of care.
Under normal circumstances, you will receive your first physiotherapy during that visit, so please wear loose-fitting, comfortable clothing that allows the physiotherapy access to the injured area.
Prior to your first visit, please download, print and complete the patient forms on this website (see below) and bring them with you.
As well, please bring any referral notes from your physician as well as a complete list of any prescribed or over-the-counter medications you take regularly.
Insurance
We accept most major insurance providers and offer direct billing services.
We also work with the Work Safety Injury Board.

Hours of operation
Monday to Friday: 10 a.m. to 7 p.m.
Saturday: 10 a.m. to 2 p.m.
